rtp/README: update pipelines to work with 1.0

- Use gst-libav encoders/decoders instead of gst-ffmpeg
- gstrtpjitterbuffer -> rtpjitterbuffer
- gst-launch-0.10 -> gst-launch-1.0
- Add 'videoconvert' element
- xvimagesink -> autovideosink
