Add a management command to lava_results_app which will allow admins
to drop all materialized views prior to migration/upgrade.

Stop looking for a value from a virtualenv and make
/etc/lava-server/instance.conf be only a default value
for the INSTANCE_NAME in /etc/lava-server/settings.conf
Allows packaging to drop the prompt asking for an instance
name.
Also describe how to update the db port on the rare
occassions when this might be useful.

lava-server from 2017.11 onwards will fail to install on a worker.
Retain the documentation on how to disable lava-server on an old
V1 worker and get to a working V2 worker installation.
