https://html.spec.whatwg.org/multipage/rendering.html#images-3 allows for the optional display of a broken image icon in the second and third cases described there. Currently we only display the icon in the second case. This CL makes us display an image icon in the third case too. This has the potential to display the icon where we've never displayed it in the past, so if authors need to have an image that doesn't display at all they will need to specify it as one that 'represents nothing' in the language of the spec. Typically they can achieve this by setting the alt string to empty, with or without a src attribute.
