Integrate latest HDMI out related patches

In addition to the bridge and VOP patches ported to the latest mainline kernel, this also contains a clean PHY driver ready for upstreaming.

The DTS patches had to be reworked - I provided the required support for EVB1 as well, hopefully I didn't miss anything.
