format "HTTP/host:port" as the third argument (pszTargetName)
to InitializeSecurityContext.  This requires adding a
host_and_port parameter to some methods.

Remove obsolete (and incorrect) logging code in
HttpNetworkTransaction::PrepareForAuthRestart().
